Attendant Care Services (ATTC) Position Title:

Personal Care Assistant

Attendant Care Services (ATTC) provides direct, hands-on support to individuals who require assistance with Activities of Daily Living (ADLs). As a Personal Care Assistant, you will play a vital role in helping clients maintain their independence, safety, and overall well-being in the comfort of their homes. You will provide compassionate, person-centered care according to each client’s individualized care plan.

Key Responsibilities:

Assist clients with personal care and ADLs, including:
  • Bathing, showering, dressing, and grooming
  • Oral hygiene
  • Nail care (excluding clients with diabetes)
  • Hair care, skin care, and shaving
  • Application of cosmetics
  • Toileting, positioning, transferring, and ambulation
  • Use of assistive devices and prescribed exercise routines
  • Feeding and meal assistance
  • Medication reminders
Support daily household tasks to maintain a safe and comfortable home environment:
  • Light housekeeping: making beds, dusting, vacuuming, mopping, cleaning bathrooms and kitchens, and laundry
  • Meal planning and preparation based on client preferences and dietary needs
  • Following written dietary plans and reinforcing special dietary instructions from physicians
Support daily household tasks to maintain a safe and comfortable home environment:
  • Escort clients to appointments, shopping trips, errands, and social outings as outlined in the care plan
  • Complete essential shopping and errands (may include handling client funds under supervisor direction)
Support daily household tasks to maintain a safe and comfortable home environment:
  • Engage clients in conversation, activities, and social interaction to promote mental and emotional well-being
  • Assist with written communication or information research as needed
Collaborate with the care team:
  • Follow the written care plan and document care provided
  • Report any changes in the client’s condition, environment, or needs to the Supervisor promptly
  • Monitor household supply levels and notify supervisor of any shortages
  • Participate in team meetings and contribute feedback to improve care delivery and client outcomes
Uphold all ATTC policies, procedures, and professional standards in service delivery

Qualifications:

  • Compassionate, dependable, and professional demeanor
  • Strong communication, interpersonal, and observational skills
  • Ability to follow detailed care plans and maintain client confidentiality
  • Prior experience as a caregiver or personal care assistant preferred
  • Certification or training as required by state regulations (if applicable)

Home & Community Assistance (HMK) Position Title:

Home & Community Assistance

Attendant Care Services (ATTC) provides essential support for individuals who need assistance with Instrumental Activities of Daily Living (IADLs). As a Companion / Homemaker, you will help clients maintain a clean, safe, and supportive living environment while promoting comfort, independence, and emotional well-being. Services are delivered according to each client’s individualized care plan.

Position Title:

Structured Family Caregiving (SFC)

Structured Family Caregiving (SFC) is a program that allows family members or other live-in caregivers to receive compensation for providing daily care and support to an individual in need. Caregivers may be related or unrelated but must reside full-time in the same household as the person receiving care. SFC recognizes and supports the value of informal caregivers by offering a paid, structured alternative to traditional home care services.

Key Features:

  • Caregivers must live full-time in the same home as the individual receiving care
  • Care may be provided by family members (e.g., parents, adult children, siblings) or non-relatives
  • Caregivers are responsible for providing both:
    • Attendant Care (ATTC) – support with Activities of Daily Living (ADLs)
    • Homemaker (HMK) tasks – support with Instrumental Activities of Daily Living (IADLs)
  • SFC caregivers perform the same duties as external, paid caregivers
  • There is no fixed schedule; caregivers clock in and out once per day at any time
  • Caregivers receive a flat daily stipend seven days a week, regardless of hours worked
  • Payment is based solely on the assessed Level of Care, not on time logged

Assessment & Payment Structure:

  • Levels of Care (Level 1, 2, or 3) are determined through a formal assessment conducted by a state-designated case manager or coordinator
  • Currently, these assessments are administered by MAXIMUS
  • Reassessments are conducted annually to ensure accurate placement
  • Payments are:
    • Based on the client's assessed level of care
    • Tax-exempt income
    • Do not impact eligibility for public benefits such as SSI, SNAP, or Medicaid

Responsibilities of SFC Caregivers:

  • Provide consistent, daily care and supervision in alignment with the individual’s needs
  • Assist with personal care (e.g., bathing, dressing, grooming, mobility support)
  • Support daily household tasks (e.g., cleaning, meal preparation, laundry, errands)
  • Follow the individual’s care plan as directed by the care coordinator
  • Maintain open communication with agency staff and participate in required check-ins or documentation
  • Ensure a safe, stable, and supportive home environment
  • Maintain certification or training as required by state regulations (if applicable)
