This is Indiana’s Medicaid program that provides services to individuals who need care at home rather than in a nursing facility. The waiver is also available to medically complex children. It replaced the Aged & Disabled (A&D) Waiver on July 1, 2024.

Individuals applying for the waiver must:

• Be eligible for Indiana Medicaid
• Require the same level of care as someone in a nursing home.

• In-home personal care assistance
• Meals delivered to your home
• Home modifications (like ramps)
• Medical Devices (like recliner lift chairs and automatic pill dispensers)
• Caregiver support
• Emergency response systems
• And more, based on your needs

• Call your local state-entity (Maximus)
• They'll do a quick phone screening to request a Nursing Facility Level of Care (NFLOC) Assessment.
• You'll wait to hear back from Maximus who will schedule and complete the NFLOC Assessment.
• If you qualify, you'll be added to the waiting list.

• Yes, it's a state list, and it's long; thousands are waiting as of mid-2025
• Priority is given to people leaving nursing homes or hospitals
• Others are added based on their assessment date

• You'll get a letter and phone call from CoAction or your local AAA
• A counselor will meet with you in person for:
  • A new level of care assessment
  • Develop a service plan
  • Choose a provider {that's us!}
• You'll receive a personal care plan
• Services begin after the plan is approved and the authorization is sent to the provider.
